Internuclear ophthalmoplegia (INO) is a disorder of eye movements caused by damage to the heavily myelinated nerve tracts known as the median longitudinal fasciculi (MLF). The MLF is essential for coordinating eye movements between the oculomotor nerve (III) nucleus on one side and the abducens nerve (VI) nucleus on the other, allowing for conjugate gaze (both eyes moving together in the same direction). 

A lesion in the MLF results in impaired adduction (inward movement) of the eye on the side of the lesion during horizontal gaze to the opposite side. The abducting (outward-moving) eye typically develops nystagmus (involuntary eye movements).
