Hypnosis in Pediatric Dentistry

Hypnosis: An altered state of consciousness characterized by heightened suggestibility, focused attention, and increased responsiveness to suggestions. It is often used to facilitate behavioral and physiological changes that are beneficial for therapeutic purposes.

  • Use in Pediatrics: According to Romanson (1981), hypnosis is recognized as one of the most effective nonpharmacologic therapies for children, particularly in managing anxiety and enhancing cooperation during medical and dental procedures.
  • Dental Application: In the field of dentistry, hypnosis is referred to as "hypnodontics" (Richardson, 1980) and is also known as psychosomatic therapy or suggestion therapy.

Benefits of Hypnosis in Dentistry

  1. Anxiety Reduction:

    • Hypnosis can significantly alleviate anxiety in children, making dental visits less stressful. This is particularly important for children who may have dental phobias or anxiety about procedures.
  2. Pain Management:

    • One of the primary advantages of hypnosis is its ability to reduce the perception of pain. By using focused attention and positive suggestions, dental professionals can help minimize discomfort during procedures.
  3. Behavioral Modification:

    • Hypnosis can encourage positive behaviors in children, such as cooperation during treatment, which can reduce the need for sedation or physical restraint.
  4. Enhanced Relaxation:

    • The hypnotic state promotes deep relaxation, helping children feel more at ease in the dental environment.

Mechanism of Action

  • Suggestibility: During hypnosis, children become more open to suggestions, allowing the dentist to guide their thoughts and feelings about the dental procedure.
  • Focused Attention: The child’s attention is directed away from the dental procedure and towards calming imagery or positive thoughts, which helps reduce anxiety and discomfort.

Implementation in Pediatric Dentistry

  1. Preparation:

    • Prior to the procedure, the dentist should explain the process of hypnosis to both the child and their parents, addressing any concerns and ensuring understanding.
  2. Induction:

    • The dentist may use various techniques to induce a hypnotic state, such as guided imagery, progressive relaxation, or verbal suggestions.
  3. Suggestion Phase:

    • Once the child is in a relaxed state, the dentist can provide positive suggestions related to the procedure, such as feeling calm, relaxed, and pain-free.
  4. Post-Hypnosis:

    • After the procedure, the dentist should gradually bring the child out of the hypnotic state, reinforcing positive feelings and experiences.

Polycarbonate Crowns in Pedodontics

Polycarbonate crowns are commonly used in pediatric dentistry, particularly for managing anterior teeth affected by nursing bottle caries. These crowns serve as temporary fixed prostheses for primary teeth, providing a functional and aesthetic solution until the natural teeth exfoliate. This lecture will discuss the indications, contraindications, and advantages of polycarbonate crowns in pedodontic practice.

Nursing Bottle Caries

  • Definition: Nursing bottle caries, also known as early childhood caries, is a condition characterized by the rapid demineralization of the anterior teeth, primarily affecting the labial surfaces.
  • Progression: The lesions begin on the labial face of the anterior teeth and can lead to extensive demineralization, affecting the entire surface of the teeth.
  • Management Goal: The primary objective is to stabilize the lesions without attempting a complete reconstruction of the coronal anatomy.

Treatment Approach

  1. Preparation of the Lesion:

    • The first step involves creating a clean periphery around the carious lesion using a small round bur.
    • Care should be taken to leave the central portion of the affected dentin intact to avoid pulp exposure.
    • This preparation allows for effective ion exchange with glass ionomer materials, facilitating a good seal.
  2. Use of Polycarbonate Crowns:

    • Polycarbonate crowns are indicated as temporary crowns for deciduous anterior teeth that will eventually exfoliate.
    • They provide a protective covering for the tooth while maintaining aesthetics and function.

Contraindications for Polycarbonate Crowns

Polycarbonate crowns may not be suitable in certain situations, including:

  • Severe Bruxism: Excessive grinding can lead to premature failure of the crown.
  • Deep Bite: A deep bite may cause undue stress on the crown, leading to potential fracture or dislodgment.
  • Excessive Abrasion: High levels of wear can compromise the integrity of the crown.

Advantages of Polycarbonate Crowns

Polycarbonate crowns offer several benefits in pediatric dentistry:

  • Time-Saving: The application of polycarbonate crowns is relatively quick, making them efficient for both the clinician and the patient.
  • Ease of Trimming: These crowns can be easily trimmed to achieve the desired fit and contour.
  • Adjustability: They can be adjusted with pliers, allowing for modifications to ensure proper seating and comfort for the patient.

  • Purpose: Laser fluorescence device used to detect early carious lesions, especially in pits and fissures.

2. Radiographic Series

  • 8-Film Series typically includes:

    • 2 bitewing radiographs (detect interproximal caries)

    • 2 occlusal radiographs (evaluate anterior teeth and jaw development)

    • 4 periapical radiographs (assess root and surrounding bone)

  • Purpose: Comprehensive view of dental and skeletal development, pathology, and trauma.

Conditioning and Behavioral Responses

This section outlines key concepts related to conditioning and behavioral responses, particularly in the context of learning and emotional responses in children.

1. Acquisition

  • Acquisition refers to the process of learning a new response to a stimulus through conditioning. This is the initial stage where an association is formed between a conditioned stimulus (CS) and an unconditioned stimulus (US).
  • Example: A child learns to associate the sound of a bell (CS) with receiving a treat (US), leading to a conditioned response (CR) of excitement when the bell rings.

2. Generalization

  • Generalization occurs when the conditioned response is evoked by stimuli that are similar to the original conditioned stimulus. This means that the learned response can be triggered by a range of similar stimuli.
  • Example: If a child has a painful experience with a doctor in a white coat, they may generalize this fear to all doctors in white coats, regardless of the specific individual or setting. Thus, any doctor wearing a white coat may elicit a fear response.

3. Extinction

  • Extinction is the process by which the conditioned behavior diminishes or disappears when the association between the conditioned stimulus and the unconditioned stimulus is no longer reinforced.
  • Example: In the previous example, if the child visits the doctor multiple times without any unpleasant experiences, the fear associated with the doctor in a white coat may gradually extinguish. The lack of reinforcement (pain) leads to a decrease in the conditioned response (fear).

4. Discrimination

  • Discrimination is the ability to differentiate between similar stimuli and respond only to the specific conditioned stimulus. It is the opposite of generalization.
  • Example: If the child is exposed to clinic settings that are different from those associated with painful experiences, they learn to discriminate between the two environments. For instance, if the child visits a friendly clinic with a different atmosphere, they may no longer associate all clinic visits with fear, leading to the extinction of the generalized fear response.

Frenectomy and Frenotomy

frenectomy is a surgical procedure that involves the complete excision of the frenum and its periosteal attachment. This procedure is typically indicated when large, fleshy frenums are present and may interfere with oral health or function.

Indications for Frenectomy

The decision to perform a frenectomy or frenotomy should be based on the ability to maintain gingival health and the presence of specific clinical conditions. The following are key indications for treating a high frenum:

  1. Persistent Gingival Inflammation:

    • A high frenum attachment associated with an area of persistent gingival inflammation that has not responded to root planing and good oral hygiene practices.
  2. Progressive Recession:

    • A frenum associated with an area of gingival recession that is progressive, indicating that the frenum may be contributing to the loss of attached gingiva.
  3. Midline Diastema:

    • A high maxillary frenum that is associated with a midline diastema (gap between the central incisors) that persists after the complete eruption of the permanent canines.
  4. Mandibular Lingual Frenum:

    • A mandibular lingual frenum that inhibits the tongue from making contact with the maxillary central incisors, potentially interfering with the child’s ability to articulate sounds such as /t/, /d/, and /l/.
    • If the child has sufficient range of motion to raise the tongue to the roof of the mouth, surgery may not be indicated. Most children typically develop the ability to produce these sounds after the age of 6 or 7, and speech therapy may be recommended if issues persist.

Surgical Considerations

  • Keratinized Gingiva:

    • If a high frenum is associated with an area of no or minimal keratinized gingiva, a vestibular extension or graft may be used to augment the surgical procedure. This is important for ensuring stable long-term results.
  • Frenotomy vs. Frenectomy:

    • In cases where a frenotomy or frenectomy does not create stable long-term results, alternative approaches may be considered. Bohannan indicated that if there is an adequate band of attached gingiva, high frenums and vestibular depth do not pose significant problems.
  • Standard Approach:

    • The use of surgical procedures to eliminate the frenum pull is considered a standard approach when indicated. The goal is to improve gingival health and function while minimizing the risk of recurrence.
